Our goal is to improve your health and well-being by connecting you to your community.

We prioritise your dignity and independence. We are here to listen and support you. Together with a Community Connector you can discuss what you need and what is important to you. We can help you find local activities and services that address your needs and then support you to access the ones that you choose.

How to Get Connected?

You can also request a referral from your general practitioner, or another healthcare provider.

Connecting Communities to Care service providers and clients

What does the program involve?

This is a free program linking you to local services and social activities so that you can keep enjoying what is important to you.
A Community Connector is a trained professional with local knowledge to support you to explore social services or activities that you might like to take part in.
In your first session with a Connector, they will learn about you and what is important to you. Together, you will identify goals and create plans to achieve them.
The Connector will ensure that you feel confident and safe to attend the services, groups or activities that you choose.
In later sessions, the Connector will support you to review your goals and reflect on your experiences.
Everything that you discuss with your Connector will be kept confidential.
Once you are connected to activities in your community, you may no longer need support from your Connector, but you are able to contact us for support at any time.
